Role Description
This is a part-time hybrid role for a Mobile Primary Care Nurse Practitioner located in Tampa, FL, with some work from home flexibility. The Nurse Practitioner will be responsible for providing high-quality and compassionate care to patients in ALF, group homes, and private homes. Duties include chronic care management, transitional care, and collaborating with community partners to enhance patient outcomes.
